9. Trivalent Influenza Vaccine: This vaccination service is applicable to individuals aged 6 months or above (age as at the date of vaccination).
10. “Left Flu, Right Lung” - Trivalent Influenza Vaccine + 20 - Valent Pneumococcal Conjugate Vaccine (Prevenar 20): This vaccination service is applicable to individuals aged 18 or above (age as at the date of vaccination). The fee includes doctor's consultation fee of $300.
11. HPV Vaccine (9-in-1) (3 doses): This vaccination service is applicable to persons aged 15 or above. The fee includes doctor's consultation fee of $300.
12. HPV Vaccine (9-in-1) (2 doses): This vaccination service is applicable to persons aged 9-14. The fee includes doctor's consultation fee of $300. 
13. New Generation Shingles (Recombinant, adjuvanted) Vaccine: This vaccination service is applicable to persons aged 50 or above / aged 18 or above at increased risk of herpes zoster. The fee includes doctor's consultation fee of $300.
14. Meningococcal B Vaccine: This vaccination service is applicable to persons aged 2 months or above. The fee includes doctor's consultation fee of $300 and single dose of Meningococcal B Vaccine.
15. Hepatitis A Vaccine: To determine the suitability for vaccination, a test report for hepatitis A/B antigen and antibody issued within 3 months of vaccination must be presented prior to vaccination. Extra fee for blood test will be charged if a valid test report is unavailable. 
16. Hepatitis B Vaccine: This vaccination service is applicable to persons aged 16 or above. To determine the suitability for vaccination, a test report for hepatitis A/B antigen and antibody issued within 3 months of vaccination must be presented prior to vaccination. Extra fee for blood test will be charged if a valid test report is unavailable. 
17. Hepatitis A&B (Combined) Vaccine: This vaccination service is applicable to persons aged 16 or above. To determine the suitability for vaccination, a test report for hepatitis A/B antigen and antibody issued within 3 months of vaccination must be presented prior to vaccination. Extra fee for blood test will be charged if a valid test report is unavailable.
18. 15 - Valent Pneumococcal Conjugate Vaccine (VAXNEUVANCE™): This vaccination service is applicable to individuals aged 6 weeks or above. The fee includes doctor's consultation fee of $300 and single dose of 15-Valent Pneumococcal Conjugate Vaccine.
19. 20 - Valent Pneumococcal Conjugate Vaccine (Prevenar 20): This vaccination service is applicable to high-risk individuals aged 18 or above or individuals aged 65 or above. The fee includes doctor's consultation fee of $300. 
20. AREXVY Respiratory Syncytial Virus Vaccine Recombinant, Adjuvanted: This vaccination service is applicable to individuals aged 60 or above. The fee includes doctor's consultation fee of $300.
21. Individuals aged below 18 (age as at the date of vaccination) MUST be accompanied by their parent(s)/guardian(s), who will sign on the consent form on behalf of the enrollees in the clinic.

23. Full payment must be settled by eligible HSBC credit cards.
24. The code ‘first 6-digit of your HSBC Credit Card+HSBC2026’ must be entered before check-out to enjoy the QHMS eShop offers.
25. This offer cannot be used in conjunction with any other privileges, corporate privilege prices, or QHMS family card and cannot be exchanged for cash or other products. 
26. Unless assessed by a doctor and diagnosed as unsuitable for vaccination, once payment is confirmed, the ordered vaccine services cannot be exchanged or refunded.
27. Applicable only to vaccines where fee includes doctor's consultation fee: Once the doctor’s assessment has been performed, the doctor's consultation fee is non-refundable regardless of the result of the suitability for vaccination. 
28. All services cannot be resold, transferred, or used for commercial purposes without explicit written consent of Quality HealthCare Medical Service Limited. 
29. Should any dispute arise, the decision of Quality HealthCare Medical Services Limited shall be final. 
30. If there is any inconsistency or ambiguity between the English and Chinese versions of these Remarks, the English version shall apply and prevail.
